TUCSON, Ariz. (13 News) – Seven people are facing charges after a patient died at a care facility in Pima County years ago.<\/p>\n

On Wednesday, June 24, Arizona Attorney General Kris Mayes announced 42 indictments for a host of cases as part of the 2026 National Health Care Fraud Takedown.<\/p>\n

\u201cThese 42 indictments \u2014 spanning fraudulent billing, drug diversion, unlicensed medical practice, and the abuse and neglect of vulnerable adults \u2014 show how tirelessly my office works to hold bad actors accountable,\u201d said Attorney General Mayes. \u201cWe will not stop until those who exploit our health care system are brought to justice.\u201d<\/p>\n

While most of the cases involved fraud, one was about the death of a patient at Meadows Catalina is located near Broadway and Sarnoff. <\/p>\n

According to the indictment, Holly Dale Given was in the care of the staff at Meadows Catalina in late 2023.<\/p>\n

Prosecutors said the staff\u2019s \u201clack of care and repeated falls resulted in the patient\u2019s death.\u201d<\/p>\n

Paula Joann Fox, Carey Jack Carrington, Maria Guadalupe Reyes-Torres, Bonith Havyarimana, Kareena Rae Torres-Hackworth, Naomi Mbazumutima, and Nevia Michelle Sander have all been indicted in Given\u2019s death.<\/p>\n

The 65-year-old Fox was charged with manslaughter and vulnerable adult abuse and her bond was set at $150,000.<\/p>\n

Havyarimana, Reyes-Torres, Carrington, Mbazumutima, Sanders and Torres-Hackworth were charged with vulnerable adult abuse. Their bonds were set at $7,500.<\/p>\n

During the initial court appearances, the judge said Given had serious injuries, including seven fractured ribs. The defense claimed it happened in September 2023. All three had bonds set at $7,500 and have arraignments at 1:30 p.m. on June 16.<\/p>\n

Another one of the cases involves 38-year-old Edgar Torres, of Nogales.<\/p>\n

He had been charged with endangerment, criminal impersonation, possession of prescription drugs for sale, and unauthorized practice of a health profession.<\/p>\n
